In keeping with the Hospital’s Mission, Vision, and Values, policies and procedures, and working within an Interdisciplinary team setting, Outreach Workers are responsible for providing community support to clients of the Mental Health Program, which enables the client to maintain an optimal level of functioning while living with a chronic mental health illness.

REQUIREMENTS:

 

    • Two (2) Year Post Secondary School Diploma in Social Service Worker or related field

 

    • Valid Class G Driver’s License

 

    • Previous experience working with individuals with severe mental illness in Renfrew County and in a community setting

 

  • Experience working as a team member

 

  • Demonstrated ability to communicate effectively and with respect in an oral and written format with patients, public, colleagues and other health disciplines

 

    • Excellent interpersonal and conflict resolution skills

 

    • Demonstrated ability and experience facilitating life skills and social recreation groups

 

    • Demonstrated knowledge of community of Renfrew County and issues concerning individuals living with serious mental illness

 

    • Demonstrated computer software application skills, i.e. Microsoft Word, Access, etc.

 

    • Excellent organizational skills and ability to set priorities

 

    • Ability to work collaboratively as a team member

 

    • Ability to coach, mentor and influence

 

    • Demonstrated knowledge of Best Practices in Mental Health and Recovery

 

    • Applicants must obtain a clear Vulnerable Sector Check

 

    • Must have demonstrated ability to meet the attendance standards of the Hospital
