|Columbus House Pembroke is currently recruiting for a Service Coordinator for the Renfrew County Coordinated Access Committee. The mandate of the Coordinated Access Committee is to implement an effective mechanism of County-wide open access to services for children, youth, and their families with complex / hard to serve mental health issues.|
This is a part-time, contract position beginning September 5, 2017. This job requires flexible hours averaging 32 hours per month. Must be available for half day meeting once a month. Attendance at other community/agency/Ministry meetings may be required. The Coordinator is responsible to the Coordinated Access Committee and the Ministry of Children & Youth.
The Coordinator is responsible for the effective management of the Coordinated Service Resolution Mechanism for Children & Youth. The Coordinator will review client data, collaborate with the referring agency, and chair/facilitate access and resolution committee (membership comprised of agencies supporting children/youth) resulting where possible in flexible and creative service responses.
