The Ottawa Valley Ontario Health Team (OVOHT) is one of 58 Ontario health teams across the province, built with the goal of delivering more connected and integrated care for patients, families, and caregivers. Under Ontario health teams, hospitals, physicians, home care, community health, and social service providers work as one coordinated and integrated team, no matter where they provide care.
OVOHT is recruiting for an Integrated Care Project Manager. This is a full-time contract opportunity until March 31, 2027
Under the direction of the strategic implementation lead, the Integrated Care Project Manager will work closely with OVOHT partners to advance and guide the development and implementation of initiatives. This position will support the reshaping and co-design of healthcare and improve health and well-being outcomes. This role will also support the reshaping and co-design of the care experiences of patients and caregivers, including a focus on primary care, and health human resource recruitment.
Responsibilities:
- Initiate, plan, and manage assigned strategic projects, including the development of scope, goals, and objectives.
- Develop and implement project charters, work plans, project schedules, change management and communication strategies, and cost estimates.
- Oversee the implementation of resources and technologies, and changes in practices and processes in alignment with a quality improvement philosophy.
- Support systems planning and the implementation of priority projects as required, including health human resource planning and recruitment, eReferral, and online care planning platforms.
- Work closely with the performance measurement and data team members to ensure evaluation plans are in place and data is being collected to inform Ontario health team metrics.
- Develop and implement an engagement plan to connect with key collaborators.

- University degree in a health discipline, public administration, or a related discipline
- Project management experience with demonstrated ability to lead collaborative projects involving stakeholders from multiple organizations and individuals with lived experience
- Experience in a healthcare setting is a strong asset
- A proven track record of being able to deliver on project and program targets
- The ability to identify and use health-related data to support planning in a population health management framework
- Demonstrated understanding of the Renfrew County healthcare system, sectors, and the social determinants of health
- Demonstrated openness and ability to foster positive relations and work with a broad range of partners
- Sound judgment, flexibility, creativity, and the ability to balance competing priorities, changing situations, and the needs of various stakeholder groups
- Demonstrated advanced computer skills (Microsoft Office, PowerPoint, Word, Excel, virtual meetings, Zoom, Teams).
Additional Requirements:
- Excellent interpersonal skills, with a track record of engaging health system leaders
- A self-starter with outstanding customer service
- Able to communicate effectively and concisely, orally and in writing, including effective presentation skills
- An understanding of healthcare delivery in rural and remote settings.
- Established Renfrew County contacts or networks that could support recruitment efforts are highly desirable
- An excellent understanding of Ontario’s healthcare system and administration.
- Excellent problem-solving, negotiation, issues management, organizational, project management, decision-making making and analytical skills
- Valid driver’s license with access to your own vehicle (travel may be required across the OVOHT region)
