The West Champlain Family Health Team (WCFHT) delivers comprehensive, team-based primary care to our community through collaboration between physicians and allied health professionals. Our focus is on patient-centred care, chronic disease management, health promotion, and timely access to care close to home.

We are currently seeking a full-time Registered Dietitian to join our on-site interdisciplinary team. This is a full-time position.

The Registered Dietician plays a vital part in supporting the health and wellness of patients through individualized nutrition care, chronic disease management, and health promotion programming.

Key Responsibilities:

  • Provide comprehensive nutrition assessments, counselling, and care plans for patients, including those with chronic diseases such as diabetes, hypertension, and cardiovascular conditions.
  • Collaborate closely with physicians, nurse practitioners, and other team members to deliver coordinated, evidence-based care.
  • Support chronic disease management programs and contribute to program development and evaluation.
  • Participate in quality improvement initiatives and team meetings.
  • Maintain accurate and timely documentation in the electronic medical record (EMR).
  • Stay up to date on best practices and contribute to knowledge sharing within the team.
Qualifications
  • Bachelor’s degree in food and nutrition or equivalent.
  • Completion of an accredited dietetic internship.
  • Registered and in good standing with the College of Dietitians of Ontario.
  • A minimum of 2 years of experience in a community or primary care setting is preferred.
  • Certified Diabetes Educator (CDE) designation is an asset.
  • Proficiency in French is an asset.
  • Strong skills in motivational interviewing, behaviour change counselling, and patient education.
  • Excellent communication, interpersonal, and organizational skills.
  • Familiarity with EMR systems (e.g., Telus PS Suite) is preferred.
